Our application form asks for your name, email address, phone number, performance and wellness goals, and availability to train. We also receive information you choose to share when you contact us. Please keep initial inquiries general; detailed medical records and payment information are not needed in this form.

How information is used

We use inquiry information to review your request, discuss coaching suitability and availability, respond to you, and arrange next steps. Submitting an application does not book an appointment or enroll you in coaching.

This website is hosted by Squarespace. Form submissions are processed through Squarespace and delivered to our business email.
